Celiachia: La pillola di Fasano (AT-1001)

I propose a video in which Dr. Fasano outlines the situation on the pill AT-1001, and in the USA is being tested for celiac disease.

As already mentioned in the post " pill for celiac disease: where is the trial? , "there are no major breakthroughs in this regard.

The human trial is in fact still in phase 2. Recall that the

Dr. Fasano is an Italian researcher, pediatrician and physiologist who has worked for many years in the U.S., and that nine years ago he discovered zonulin, a protein that would cause a high permeability bowel and, in genetically predisposed individuals, and trigger the autoimmune inflammatory process of celiac disease.

Havana celebrates its 490 years

Havana, officially founded November 16, 1519, the then Governor of Diego Velazquez, with the name of Villa de San Cristóbal de La Habana, is today taking the venerable age of 490 years!
The anniversary will be celebrated with the inauguration of buildings restored historic, concerts, sporting events and the traditional "ceremony" to the Temple, where, 490 years ago, according to tradition, under the Ceiba tree, the first mass was celebrated and chose the first "town council", the local government composed of representatives of the three powers of the English military, civil and religious.
Every year, from midnight on November 15 at midnight on 16 for 24 consecutive hours, on the occasion of the birthday of Havana, long lines of people, including many tourists, they go under the ceiba tree in the Template for a ancient ritual that is lucky to make three slow laps counterclockwise around the trunk, touching without speaking with three wishes, one for each lap completed.
course, is not the original ceiba nearly five centuries ago, but it's a young tree planted in 1959.

Celiachia: in aumento le diagnosi errate

Professor Umberto Volta , Chairman of the Scientific Committee of micr AIC (Italian Celiac Association), in recent times, is raising the problem of misdiagnosis of Celiac [1] .

In short, on the one hand, the Celiac disease is a diagnosis that doctors are very many still consider the other, it appears that many diagnoses of celiac disease are not really correct.

This would involve other diseases.

For those involved in autoimmune disease, is not new: such diseases, by their nature, are generally easy to diagnose. We must in fact have considerable experience to make a diagnosis called "differential".

Let's see, in short, why.

First, not you can be sure if the disease is only part of the remission of symptoms after gluten-free diet.

One of the biggest mistakes you can commit is the exclusion of gluten-free "testing." Many conditions can benefit from the elimination of gluten which, remember, is a food and glue can be difficult to digest.

As we know, there are other diseases in which doctors prefer to exclude (if only to caution) gluten, but these are forms of celiac disease.

Another problem is that the flattening of the villi. Not only does the Celiac causes. But antibiotics intestinal infections or other diseases.

In conclusion, the diagnosis should be carried out by hospitals or facilities that are able to actually see if it comes to Celiac Disease or other diseases.

This is done either through knowledge of the diagnostic protocol, is counting on being inclusive, it is also essential to know the symptoms or non-standard situations, such as extra-intestinal symptoms with celiac disease or celiac disease sero-negative.

However, with proper precautions, I would emphasize two aspects.

The first is that to exclude celiac disease does NOT mean leaving the patient without a diagnosis, which unfortunately often happens.

If you experience a screening of first degree relatives of celiac disease patients based only on genetic predisposition, the protocol we would exclude the possibility, in principle, subject to other controls on HLA negative.

The data so far known, people with celiac disease were HLA negative 9% of the total population celiac, if we look at the figures in this way does not seem like much, but we're still talking about 9 of every 100 people.

Practically, 1 in 10 people.

How to move and work in Cuba?

Due to the demand, traquillizziamo readers that despite lapse of more than one year after publication, still apply the indigo contained in the post "How to move and work in Cuba? " , which brought forward, making small corrections.

(Resident Abroad), arriving in Cuba with his spouse, may extend the visa familiar A-2, every 3 months up to a maximum of 11 months and 29 days, provided that you have sufficient income to live almost a years without working.

order to live and work in Cuba there is a need or "Permanent Resident" or "Residence Time"
. Speaking before the
"Permanent Residency" list should be at the Embassy in Italy, presenting a form, filled in English and in all its parts, which must attach the following documents: 1) birth certificate, 2 ) certificate residence, 3) chest radiography, and serological testing of AIDS, made by no later than 6 months, 4) Certificate of technical skill and practice, for over 16 years and under 60, may be certified for study, employment or retirement, 5) Certificate of criminal record, issued no more than 3 months, 6) Bank references, 7) motivation letter with the request, written in English, 8) Two passport photos, 9) Photocopy of passport in the consulate to be authenticated by showing the original.
The first five documents to be legalized in the prefecture, translated into English and legalized in the Cuban embassy.

The granting or not of the "Permanent Residency" will be communicated between 3 and 6 months from the date of application. If it will be issued an A-2, for a period of 30 days.

Different is the case of "Temporary Residence" , which is awarded for study or work. In this case, the company that takes through the body of Cuban membership, or the University to them requires the Department of Immigration.
Find a job, honestly, is not so simple. Personally it took me almost a year. A beginning may be to contact foreign companies, perhaps by going to the Chamber of Commerce of Cuba , and of course have the appropriate skills, provided they are willing to take. There are also some companies or institutions which take Cuban nationals, such as in my case and Stefano. Almost all universities in Cuba, also depends on the student's age, offer the opportunity to enroll the entire degree course or only to specific materials. The validity of the "Temporary Residence" is linked to the duration of the contract of employment or studies.
To open a business in Cuba, the first suggestion is to make an appointment note the details of our embassy in Havana Commercial, which may provide useful clarification of the law or to contact the ' ICE Office of City Mexico , which is responsible for Cuba, of course, in this case, it may be helpful to contact the Chamber of Commerce of Cuba . The weekly Opciones as needed, the public demands of the Cuban companies seeking partners for imports or exports. Another suggestion would be to visit the International Fair of Havana 2009, which runs from November 2nd to 7th, you never know ...

It 'clear that in any case, we need capital, expertise and have a valid project to be submitted to the Cuban authorities.

The Gran Hotel Havana Trotcha

Da questa data si dedicò, invece, alla compravendita di terreni dove costruiva lussuose residenze da sell to the wealthy families of Havana who were moving gradually toward the Cerro El Vedado.
And on land located at the intersection of two roads and Calzada, acquired by the Count of Pozos Dulces, built for his family a luxurious residence near the sea.
In 1886, the works were completed, Catalan, however, thought to transform the gardens and some rooms at the "Sociedad Vedado" , he fondada, serving the best dishes in the kitchen English, Creole and French. In 1890 Don Buenaventura
adapted the upper floors for hotel, giving it 20 rooms, one suite, health services, shared bathrooms, kitchen and bar-restaurant, all surrounded by beautiful gardens. The roof was wood shingles and French while the exterior windows were glazed with polychrome wrought iron railings. She was born cos ¡ the "Gran Hotel Trotcha" .
In 1902 the Catalan decided to expand the hotel and building a new lock on a ground nearby. The building, two floors of wood, was called El Eden .
was expanded again in 1904 with the construction of the Washington , a brick structure three stories high.
The Grand Hotel Trotcha worked until the crisis of the thirties, when it was converted into a guest house with 60 rooms. In 1986, a fire destroyed completely, leaving only a few remains.
The ashes of the old hotel, it is appropriate to say it should be interesting, while keeping the facade built in 1886, the new Hotel Trotcha. The competition for the award of projects, of which we can see an elaborate below, was won in 2007 by architects Cuban José Antonio Choy and Julia León .

Celiac disease is transmitted to children? How? eredit

If you did, a doctor, a question like that, I guess that the answer was not very understandable.

Me too, until I studied a bit of genetics at the university, I had very clear ideas.

speech is actually a fairly simple and involves genes, but not limited to those.

As we saw in the post and Celiac Disease HLA , even if you possess the genes called "typical" of celiac disease, That does not mean that the disease has been unleashed, as well as not to get to 100% does not rule out that there could be ill.

There is a first major subdivision of genetic diseases:

  • monogenic (single gene mutation)
  • multifactorial (non-Mendelian or complex, in which there are many genes and is stronger than the influence of environmental).

In the former, the presence of a single gene is absolutely a given diagnosis. Not only that. The diagnosis can be performed at birth (or even prenatal).

It 's the case of diseases such as thalassemia (or Mediterranean anemia), o meno gravi, come l'albinismo.

Coeliac disease and autoimmune diseases such as autoimmune thyroiditis, rheumatoid arthritis, Crohn's disease, etc. .. are multifactorial diseases.

Inherit the Celiac or not, is not currently expected, since this disease does not follow the Mendelian rules.

Carry an intervention by Prof. Maria Cristina Mazzilli (Professor of Genetics, La Sapienza University in Rome) to the AIC Conference on Celiac Disease of 07/03/2009 [2] [3].

Professor speaks in the first part of how to interpret the data and HLA in the second, provides the results of a search, always on HLA.
In summary, research reports that HLA covers only 40% of cases of disease while other HLA genes may play a role in the development thereof.

In this study, a cohort of children and controls, it is evidenced that:

  • 81% of these cases had the DQ2 DQ8
  • 10% 9% this
  • neither the one nor the other.

Of this 9% (nearly 1 in 10), 6% dimer only the DQB1 * 02, 2% had only the dimer DQA1 * 05, while the remaining 1%, none of the known dimers.

The risk, therefore, made up by having a positive HLA consists in 'having about 51% more likely to contract the disease than those without predisposing genes.

And, as we have seen, have a gene does not mean that it is "active".
gene activity is given by the adjustment mechanism. And
'This is the mechanism currently most studied in genetics.

Vaccino influenza A (H1N1) e Celiachia

Here the text of the statement of Professor Umberto Volta, CSN President vaccino2 AIC (Italian Celiac Association), concerning the problem of vaccination against influenza A for people with celiac disease.

This notification is given in full by the official website of the Italian Celiac disease [1].

"The advice that, as president of the CSN-MA, I would give the gluten is to get vaccinated in the presence of underlying conditions that increase the risk of infection, poor response to the diet on the clinical and in the presence of risk factors related to age, pregnancy and work- (tutti indipendenti dalla celiachia), mentre per la stragrande maggioranza dei celiaci a dieta aglutinata stretta la decisione di vaccinarsi rimane del tutto personale in quanto il rischio di contrarre l'infezione suina è paragonabile a quello della popolazione generale non celiaca.

analisi2 As you know, this blog is an operation from patient to patient, and born of the belief that to move in the celiac condition, or suspect that both must share the maximum information.

The purpose is not to cure us alone, as some doctors fear often suspicious sites, forums and blogs like this.
But, conversely, to understand what is the right way to arrive at a diagnosis and then live with this disease.

E 'then why we stress that after the diagnosis, attention should not decrease.
Analysis, and visited not be exhausted by it.

There is a protocol, called "follow-up [1]" to be followed by the newly-diagnosed celiac disease and that the centers of excellence (hospitals which, along with high experience on this disease, you also have a team of doctors of various specializations) are running.

The Protocol was born from the work of the Scientific Committee Country AIC, Volta and co-ordinated by Prof. Prof.Greco and controlled by committees of national experts and ministry.

born not only for the follow-up, but also for diagnosis.

The protocol for follow-up for celiac disease [2]:

to run:

  • Verifica correttezza della dieta (errori involontari)
  • Complicanze per la diagnosi in età adulta
  • Insorgenza di patologie autoimmuni associate
  • Complicanze metaboliche o neoplastiche

Come si svolge:

  • Primo controllo a 6 mesi dalla diagnosi.
  • Successivi a 1-2 anni

ferritina, emocromo, acido folico, altri test di assorbimento, antitransglutaminasi (TTG) IGA (IgG in cases of IgA deficiency) values \u200b\u200bfor thyroid, but not only TSH, T3 and T4, also of specific antibodies of autoimmune thyroiditis, and thyroid peroxidase antibodies (AcTPO), thyroglobulin antibodies (ACTG)

Control Level II

are carried out also controls metabolism.
As often about 2 years from the diet can be a syndrome of fatty liver, as well as ultrasound of the liver transaminases you do the analysis.
immunological tests.
tests to check that there are excessive increases in body mass, due to better absorption or psychological compensation in the amount of food.
Densiometria bone (In adults).
thyroid ultrasound and abdominal and / or barium enema examinations are useful.

When you do not make the second endoscopy with multiple biopsies after a period of diet, except for special cases.

Clinical improvement (reduction and absence of symptoms) and examinations will not make this practice necessary.

Colon irritabile e Celiachia

in the diagnosis of celiac disease, a very important role is covered with a disorder called Irritable Bowel. " irritablebowel

irritable bowel syndrome is not a disease but a symptom, a disorder of intestinal function properly, which can have several causes.

Unfortunately, very often, the symptoms of celiac disease is confused, even by physicians, with those of irritable bowel syndrome.

The correct name is "Irritable Bowel Syndrome", indicated by the acronym IBS (Irritable Bowel Syndrome).

E 'misuse the term "colitis" because it is not an inflammation of the colon (colonoscopy the colon is intact, without flogosi nè edemi).

Fra i compiti Colon was undoubtedly to absorb from food (already digested by the other parts of the digestive tract), particularly liquids.

It follows that the excessive digestion of the stasis of these differences in this stretch, due to poor motility and dries the feces, causing constipation.

In some cases you can have diarrhea, but may be due to the growth of bacteria and gases that caused the stagnation of faeces.

Symptoms

First, it must be said that the diagnosis of IBS is made to "exclusion" [2] . Indeed

well is essential to analyze the symptoms and make the first analysis of diseases with similar symptoms. Only if these are negative, you can talk about Irritable bowel syndrome and initiate therapy.

The symptoms of IBS are exclusively intestinal

  • constipation (constipation) recurring
  • sporadic diarrhea after periods of constipation or alternating constipation
  • diffuse abdominal pain
  • abdominal swelling (diffuse or localized in the left side and bottom of 'abdomen)

pains not to be confused with heartburn (gastritis).

IMPORTANT: Under no circumstances Irritable bowel syndrome causes anemia or malabsorption.

These symptoms should not be used for self as totally included in many other diseases (eg. Alterations of bacterial flora, lactose intolerance, Celiac disease, Crohn's disease, diabetes, autoimmune diseases, intestinal infections, etc. ..).

Diagnosis

currently uses the "Rome Policy" published in two versions, one in 1992 (Rome I criteria) and the other in 1999 (Rome II criteria) [3] .

This is a series of events that, if concurrent, may support the diagnosis of this syndrome.

However, recently the American Association of Gastroenterology on IBS, which has recommended should be the attitude of medical practitioners in their diagnosis, as is often diagnostic error, thus ignoring the real cause of the symptoms. Here is an excerpt

then part of Gastronet regarding these recommendations [4] :

" From practical point of view it is important a careful history supplemented by a thorough physical examination that is intended to exclude hepatomegaly , abdominal masses or signs of intestinal obstruction. It may be necessary to supplement the examination with an exploration rectal, especially when you described the presence of blood in the stool or symptoms of incontinenza.Si recommended that a full blood examination and a fecal occult blood screening as. Among the recommended tests include ESR, clinical biochemistry, TSH and stool for parasites and eggs.
For patients older than 50 years is recommended a colonoscopy, while younger subjects examination shall be conducted only with suspicion of organic disease (mainly in the differential diagnosis of IBD).
In young subjects who develop diarrhea may be helpful determining gliadin antibodies (AGA) and endomysial (EMA) for the diagnosis of celiac disease.
If initial treatment fails, the clinical picture should be studied with other imaging techniques such as rectal manometry, Breath-Test glucose or lactose for the study of transit for the study of Intestinal Bacterial Overgrowth.
In some cases it may be appropriate to obtain biopsies of the colon to rule out a collagen disease or lymphocytic colitis. "

therapy

Since it is not known because of the" real "Irritable bowel syndrome and is thought resulting from a disorder related to the nerve endings of the intestine, is often the case that doctors only give drugs antispasmodics.

Nessuna analisi, nessuna indagine, nessuna ipotesi alternativa: subito antispastici e antidepressivi.

Stefano and all his lovely family.
On the cyclones, thank God, everything is still quiet ...

How Much Do Truffles Cost

There are more! Happy Easter from Havana

There are more! Busy between work, home and future projects, I'm still here, but unfortunately with very little time to devote to the blog. A bit 'of various mishaps, but I still plan to hang the shoes al chiodo o, come nella foto, al filo. I "Pensieri Avaneri" continuano, e che pensieri!
Ci rifaremo dopo il ponte del 1 maggio.

La Quinta Durañona Marianao

along the 'Avenida 51 , former Royal Calzada Calzada de los Quemados and Marianao the corner of Calle 118, 11805 house, I was always intrigued by an impressive building of considerable architectural merit but on the front clearly mutilated, which occupies almost the entire block.

It 's the "Quinta de las figuras" , then renamed "Fifth Durañona" , built in 1860 by Jose Morales de los Rios on land bought in Don Ignacio Vildasola December 19, 1859, for a price of 8,412 crowns.
The house, then reported to the No 105 of the Calzada de los Quemados Real, was later mortgaged to Don José Sedano and changed ownership on several occasions in the years 1864, 1868 and 1869, when it was bought by Doña Antonia de Otamendi Durañona , resident Marianao in Calle del Rey (now 116) at a price of 14,737 pesos.
La Quinta, September 28, 1899, was inherited in
Nella foto in low, in fact, taken in 1900 you may notice the American flag flying on home.

The August 22, 1906 was sold by Don Francisco Marcial to Don Manuel y Parajón Cocio , a native of Asturias, at a price of 20 thousand pesos of gold of the English brand.
Also in 1906, the fraudulent re-election of the first president, Tomas Estrada Palma
, caused an insurrection and the arrival of U.S. troops that controlled Cuba until 1909. During these three years, the Fifth Durañona was the seat of government of the U.S. occupation.
On May 21, 1907, Don Manuel Parajón sold the villa to
Nicolas Ruiz y Altuzarra , English resident in Havana Calle Prado No. 66, at a price of 55 thousand pesos of gold.

As of June 29, 1913, the newly elected president, Gen. Mario Garcia Menocal , became the Presidential Palace in Summer . At that time the presidency was still housed in the old Palace of the Captains General in Old Havana. It was not yet built the Presidential Palace, now the Museum of the Revolution, which was originally conceived as the seat of the provincial government in Havana, and inaugurated in 1920 by the same Menocal. In 1919
hosted the Fifth
Sanatorium Centre Castellano . The building underwent some internal changes without changes to the style of the initial construction.
in 1937 was the site of events, meetings, charity evenings and solidarity with the English people during the Civil War, and January 1 of that year it was founded on May Day pro Local Committee.

Then it was home to a ' Manuals Academy of Arts, College of Men Claude Dumas and in 1943 the Normal School for Teachers Havana . In 1952, it housed on the ground floor, butcher "La Favorita" and a cheap restaurant, while the upper floors of a textile factory.
After the triumph of the Revolution hosted the
Museum Marianao, the Carlos Rodriguez Heels Gareaga , while the outbuildings, the provincial company of food and equipment, at the bottom, some houses.

Since 1993, houses the Centre Prodanza of Cuba, directed by Laura Alonso, who has six ballrooms, offices, dressing room, mess room, and video recordings. The villa also houses a professional and a vocational school, attended by students from Marianao, Playa and La Lisa.
The years have permanently erased and neglect the beautiful façade of the house, beyond recognition, but not its centuries-old history.
